Prepare for Your Visit

You will receive a courtesy reminder call two days prior to your appointment. Please confirm or cancel at that time. Also be advised that we do charge a $50.00 fee for No Show or Missed appointments.  

When arriving at the office please come to the front desk of the reception area.  There is a “sign-in” sheet at the front desk.  If you don’t sign in, we will not know that you have arrived for your appointment!

If this is your first visit, there will be some paperwork for you to fill out.  You may download and fill out the paperwork from our website before you arrive (click the forms tab above).  If you do so, please give them to the receptionist when your name is called.

After you sign in, have a seat and wait for a member of the front desk staff to call for you.  At that time you may present your paperwork or you will be asked to complete our registration documents at that time and supply us with a current copy of your insurance card. If you do not have your insurance card, you will be asked to reschedule your appointment.

Self-pay patients will be asked to make their payment upon registration.  After completion of the registration information you will be asked to complete a health history form.  Please be sure to bring along with you a list of your medications.  When your paperwork is complete, please have a seat and one of our Medical Assistants will call you for your appointment.

We will try to keep you up to date on any delays and ask for your patience:  a surgical clinic often has many unexpected delays and emergencies.  Please respect that we give all of our patients the time required to handle their problems, and remember that you would like the same courtesy extended to you.

There are usually many physicians seeing patients at the same time.  This may result in people registering after you being seen first.  In this case they are probably seeing another doctor, and you have most likely not been passed over.
